NVIDIA display driver crash

I have a vista and I started to get blue screen crash and when I turned it back on and windows automatically started looking for a solution to the problem... it said the problem was that "your display driver has crashed and windows shutdown to prevent damage"... so after the search for a solution the only solution it found was to install another display driver from NVIDIA website... so I did this but it's still crashing for the same reason.

Im having a problem with my video adapter, I reformatted my PC, installed a nvidia geforce4 mx440 with agp8x without hassle, but when I reboot my computer the video card driver was not installed, I need to re-install the drive. why is it that when I re-boot my computer, I have to re-install my video card? any help will be great. Please.
